New Client Information


As a new client, finding a new veterinarian for your pet can be a difficult decision. As part of the process
 in easing the transition, we ask that new clients provide us with current contact information and information
on their pet(s). Faxing or emailing this information prior to scheduling your pet(s) veterinary appointment or
boarding stay, will make your first visit to Bayside Animal Hospital an easy one.
 

Boarding Agreement


Having the boarding agreement signed prior to arrival for your pet(s)' stay will delay your check-in time.
Please allow 15 minutes when checking in your pet(s) for their boarding stay, so that we may verify feeding,
medicating, grooming and playtime instructions.
